en: The objective of research was to determine diagnostic efficacy of urinary concentrations of leucine-rich alpha-2-glycoprotein (A2GRL) in patients with acute appendicitis. There were included patients with high suspicion of acute appendicitis (group A; cases, n = 30). In group B (controls, n = 30) were included patients with non-surgical abdominal pain who attended to Hospital Central “Dr. Urquinaona”, Maracaibo, Zulia. General characteristics, urinary concentrations of A2GRL and diagnostic efficacy of assay were evaluated. Mean age of patients in group A was 36.3 +/- 8.8 years-old and in group B was 35.8 +/- 9.6 years-old (p = ns). There were not found significant differences in sex distribution, weight and height between groups (p = ns). Urinary concentrations of A2GRL were significantly higher in group A (1543.8 +/- 762.7 ng/mL) compared with mean value in patients of group B (774.1 +/- 356.1 ng/mL; p < 0.0001). A2GRL presented a value below curve 0.81. A cut-off value of 1000 ng/mL, showed sensivity 73.3%, specificity 70.9%, positive predictive value 72.4% and negative predictive value 72.4%. It is concluded that urinary concentrations of A2GRL have a high diagnostic efficacy in patients with acute appendicitis.>>>

en: Abstract (english) Insulin Resistance (IR) is an important finding in several diseases including diabetes and metabolic syndrome, and its diagnosis seems pertinent during the evaluation of insulin sensitivity, though mathematical models like HOMA (Homeostasis Model Assessment). The purpose of the present study was to determine an appropriate cutpoint for HOMA-IR in adult individuals from the Maracaibo municipality, Zulia state, Venezuela. Two-thousand and twenty-six individuals from both sexes and beyond 18 years of age were selected from the Maracaibo city Metabolic Syndrome Prevalence Study, a descriptive cross-sectional study with multietapic sampling. HOMA-IR was calculated using the formula [Fasting Insulin (μU/L) x Fasting Glycemia (mmol/L)/22,5]. To estimate the cutpoint, 602 healthy individuals were selected and a percentile distribution was calculates, alongside ROC Curve in order to identify the best cutoff point according to sensitivity and specificity. Overall, the average HOMA-IR was 3,71±3,01, with 3,65±2,96 for women and 3,76±3,06 for men (p=0,397). Using the reference population, the resulting arithmetic value was 2,64±1,67. When distributing per percentile, p75 was 3,02. When selecting a cutpoint using ROC Curve, the chosen cutoff point was 3.03 with an Area Under the Curve of 0.814 (75,2% sensitivity and 75,6% specificity). The obtained results are good enough to propose a cutpoint of 3,00 for HOMA-IR, which can be use in the clinical evaluation of IR in adults from our population.>>>

en: This research aims to model fuzzy characteristics of hemiplegic indicators in the pelvis. These indicators are consistent with those used by specialists to classify spastic hemiplegia following the classification proposed by Dr. Gage. The sample consisted of 83 patients with motor dysfunction subtype of spastic hemiplegia. These patients have been treated with protocols of Children's Orthopedic Hospital (HOI) in Caracas, Venezuela, between 1999 and 2008. Using statistical tools to indicators in pelvis, the average, the standard deviation and analysis of variance (ANOVA) are calculated. These statistics are suitable for building a fuzzy model with membership functions to discriminate types of hemiplegia comparable to the real world. If ANOVA has a value of p << 0.05; hemiplegic indicators in the pelvis are appropriate for the classification. 75% of records were processed and the remaining 25% were used to validate the results according to membership degree and sensitivity. The sensitivity obtained was 89% for type 1, 100% for type 2, 67% for type 3 and 90% for type 4. Experts said that the terms that describe the indicators pelvis, have a natural language, which allows classifying hemiplegic patients in a fuzzy way by degrees of membership.>>>

en: Deep placentation in human pregnancy is done by deep invasion of the placental bed by the extravillous trophoblast, involving the decidua and the inner (junctional zone) myometrium. Interstitial invasion of the stroma and endovascular trophoblast invasion of the spiral arteries both occur. Deep endovascular trophoblast invasion into the myometrial segments of spiral arteries is important for proper placental functioning. Decidua-associated vascular remodeling, which includes swelling and disorganization of the vascular smooth muscle, occurs during a period of rising placental oxygen. This early remodeling step may accommodate the progressively increasing maternal blood flow to the developing placenta. The subsequent trophoblast-associated remodeling step enhances and stabilizes the widening of the vessels, whereas the vascular smooth muscle and elastic lamina are replaced by a fibrinoid matrix with embedded trophoblast.>>>

en: (english ) Soft tissue sarcomas represent less than 1% of all human neoplasms. One-third of malignant tumors that arise in the retroperitoneum are sarcomas and liposarcoma is the most common retroperitoneal sarcoma and is known to grow to giant sizes, slow progress and few late symptoms. We report the case of a 40 year old woman with a giant retroperitoneal liposarcoma. A laparotomy was performed and a multilobulated tumor of 20 centimeters of diameter arising from retroperitoneum. The histological features were suggestive of pleomorphic liposarcoma weighing 8.5 Kilograms.>>>
